KINDERGARTEN

Applications for the 2012-13 school year are due February 17th

Please submit:

• Pre-registration form
• Birth Certificate, copy
• If child is not born in US we need a copy of their Visa
• Baptismal Certificate, if Catholic, copy
• Teacher Recommendation Form (coming soon)
• Testing fee of $50
• Testing needs to be scheduled individually- testing dates are February 15th- 17th and 23rd-24th. Please call Julie Blue at 214-365-1232 to schedule
• Immunization Record

1st- 8th GRADE

Applications for the 2012-13 school year are due January 27th

Please submit:

• Pre-registration form
• Birth Certificate, copy
• Baptismal Certificate, if Catholic, copy
• 1st Reconcilliation Certificate and 1st Communion Certificate, if applicable, copies
• Teacher recommendation form
• Transcipts from current school including educational and diagnostic testing, current report card and standardized achievement test results
• Testing fee of $50
• Testing is at Christ the King on January 21st from 8:30-11:30
• Immunization Record

Once all materials are received, applications will be reviewed and decisions will be made.
